Required field Certificate of Health Status* I hereby certify that my child is currently healthy enough to participate in swimming lessons. Should my child’s health condition change during the course, I am required to report this. No financial compensation will be provided for missed lessons. The legal guardian remains responsible for supervision at all times, even during the swimming course.*

Image and Video Usage Rights YES, I hereby grant Sole Felsen Welt perpetual right to use the photos and/or videos taken of my child during various swimming lessons. I agree that the photos and/or videos in which my son/daughter appears may be used for the following purposes: print, website, newsletter, and social media. This consent may be revoked at any time.
